Hi, I'm Jessica. I’m a Registered Nurse with a Master's degree in Nursing Administration, and I’ve been practicing for over 20 years. Most of my career has focused on geriatrics, with a passion for helping my patients remain in the comfort of their own homes. I specialize in coordinating care, managing complex health issues, and guiding families through the healthcare system, all while overseeing home health care providers. I’ve worked extensively with hospice patients and pride myself on providing emotional support to both my patients and their families. When I'm not working, I enjoy spending time with my family and exploring new ways to stay active, as I believe in the importance of both mental and physical well-being.

Dena Salzberg
Hi! I'm Dena, a Registered Nurse with over 40 years of experience working in Geriatrics, Public Health, and Home Care. I founded Care Management Consultants in 1999, with a strong focus on serving the unique needs of the elderly and disabled community. My professional journey has allowed me to witness firsthand the challenges faced by individuals and families, and I am deeply committed to providing compassionate, personalized care. Throughout my career, I have always prioritized the well-being and independence of my clients. I believe that every individual deserves to maintain as much autonomy as possible, which is why I work tirelessly to support my clients either by managing their care or by coordinating with skilled caregivers. My goal is to ensure that they have the resources, guidance, and care they need to live life to the fullest. As a patient and family advocate, I understand the importance of communication and trust in the caregiving process. I am here to help families navigate the complexities of healthcare and make informed decisions that truly benefit their loved ones. Whether you're looking for advice, care management, or support with finding the right caregivers, I'm here to help. I believe in providing more than just medical care—I aim to offer peace of mind for both my clients and their families. I have extensive experience advocating for my clients and am highly regarded within several community organizations. I serve on the board of the Patient and Family Advisory Councils at Mass General Brigham Hospital and have participated in various grants through the Patient-Centered Outcomes Research Institute (PCORI), focusing on issues affecting the elderly.
